Summer Activity Book Completion Form
Did your child complete a summer workbook? If so, please print our Summer Activity Book Completion Form, fill it out, and return it to your child’s teacher by Sept. 23rd. Membership Form
Please join your PTA. Your membership helps our school provide academic and social programs for our kids. We’ve already got 70% of our teachers signed up and we’d love to have that representation from our parents as well.
You do not have to attend our meetings to be a member, however you do have to renew your registration each year. So please print a Membership Application and return it to school with your child. Thank you for your support.

Adopt a Teacher Form
Our staff at FRES are AWESOME and we want to show them our appreciation and support. If you would like to “adopt” a staff member and send them special treats, supplies, and notes of thanks throughout the year, please download an Adopt A Teacher form and return it to school.
Student Directory/Parent Information Form
Each year, the PTA publishes a directory for participating families that lists students alphabetically AND by grade/teacher. If you want to be listed in the directory, you must complete the Student Directory/Parent Information Form, sign it, and check the appropriate box at the bottom of the page.
You can also use this form to provide the PTA with information about you that we will use when contacting specific groups of individuals (e.g. we might use the information you provide if we need to contact parents with children in a specific grade). We will not share this information with anyone and you can select the appropriate box at the bottom of the form if you do not want your information printed in our directory.
